Step one:

Start with a large white canvas. Make a new layer set titled Clean Glass Plate Header.

Choose the rounded rectangle tool and set radius setting to 8px.

In a new layer draw a #2DAEFFF round rectangle using the rounded rectangle tool. (width: 550 x height: 40)

Add the following layer styles: Inner Shadow, Outer Glow, Stroke.

Step two:

Choose the rounded rectangle tool and set radius setting to 4px.

In a new layer draw a #0073B9 round rectangle using the rounded rectangle tool. (width: 48 x height: 29)

Add the following layer styles: Outer Glow, Satin, Gradient Overlay, Stroke.
