Step one:
# Start with a large black canvas. Make a new layer set titled ABS Plastic Header.
# In a new layer draw a #636363 rectangle leaving blank space above your canvas.. (width: 640 x height: 48)
# Add the following layer styles: Gradient Overlay.

# Step two:
# In a new layer draw a 1px white line across the #636363 rectangle structure.
# Set the layer’s opacity level to 6%.

Step three:
# Above the white line, draw a 1px black line across the rectangle structure.
# Set the layer’s opacity level to 7%.

# Step four:
# Choose the rounded rectangle tool and set the radius setting to 20px.
# In a new layer draw a #18326F round rectangle using the rounded rectangle tool. (width: 261 x height: 15)
# Cut the top half of the round rectangle.
# Add the following layer styles: Inner Shadow, Inner Glow, Gradient Overlay, Stroke.

Step five:
# Duplicate the structure from step four and position as shown below.

# Step six:
# Make a wide rectangle in marquee selection.
# Set your foreground color to #CBCACB and background to #5B5C5C.
# Using the gradient tool, in a new layer apply a linear gradient effect on the marquee selection.

Step seven:
# Select the rounded rectangle tool and set the radius setting to 20px.
# In a new layer draw a black round rectangle using the rounded rectangle tool. (width: 542 x height: 68)
# Add the following layer styles: Inner Shadow, Stroke.

# Step eight:
# Inside the black round rectangle, clone a stock photo inside in a seperate layer.

Step nine:
# Add your website name using font Verdana, regular, 18pt, sharp and #FFFFFF.
# Add the following layer styles: Gradient Overlay, Stroke.

# Step ten:
# Now add a logo by typing “?” on your keyword with font webdings, regular, 48pt, sharp and #000000.
# Add the following layer styles: Drop Shadow, Gradient Overlay.

Final:
# ABS Plastic Header
